The essay deals with case study of Catherine Bexley, a 77 year old woman, presented to ED with right lower lobe pneumonia. The patient also has the history of heart disease, hypertension, and hypercholesterolemia. In response to the case study the essay discusses the primary assessment using the DRSABCDE approach followed by the focused assessment for respirat ory distress. The essay then discusses the pathophysiology of the shock experienced by the patient and explains the clinical manifestations. Lastly, the essay presents the appropriated nursing interventions, based on evidence to improve Catherines health outcomes. DRSABCDE approach will be used for primary assessment. It stands for Danger, Response, Send for help, Airway, Breathing, Circulation, Disability, Exposure. According to Smith Bowden (2017) it is the systematic approach to immediate assessment and treatment. On assessment of Danger, initially the patient appeared to need HDU however, found safe to ward after oxygen and antibiotics. The DRS assessment showed that patient is conscious and responsive. She was further assessed using PPE. Immediate help required in the ED is that of physician. The patient may be at risk of shock and death due to low oxygen supply. The neurological assessment showed that the patient is complaining of pain and is confused. The airway assessment showed that the Catherine is using her upper accessory muscles. She has decreased air entry to her Right lobe to midzone. There were no noisy respirations found and no sign of cyanosis. On breathing assessment, shortness of breath was found and the patient is having increased effort evident from speaking of short sentences. He patient has pain 5/10 in her Right lower chest with deep. Immediate intervention is needed to prevent to prevent decreased GCS (15/15 in patient), life threatening conditions (Velasco Howard, 2017). The circulatory assessment revealed low BP 90/65mmHg instead of normal 120/80, HR 120 bpm instead of 96 regular, The radial pulse was very weak, with skin flushed and sweaty. The capillary refill is 3 seconds. Her ECG is NAD. Immediate intervention is required as hypovolaemia, pump failure, and vasodilatation (sepsis) causes this breathing problem. Adequate perfusion is needed (Perman, Goyal Gaieski, 2012). Further patient was assessed for disability to rule out possibility of stroke. Catherine was confused to day and place. Further, assessment includes examination of patient head to toe and front and back, showed no sign of haemorrhage but the patient was bleeding as she has removed her IV line. The patient has sign of fever and is ensured to be warm. According to Zhang, Chen Ni (2015) fever, chills, confusion, difficult breathing and significant drop in blood pressure are indicators of septic shock. Thereby, focused assessment is necessary. The relevant focused assessment required in this case is related to respiratory. The red flags for focusing on respiratory assessment are chest pain and shortness of breath. Therefore, the secondary assessment is justified (Nemer Villar, 2015). The oxygen flow rate and delivery device is assessed. It is necessary as the patient demonstrated hypoxia, tachypnoea, tachycardia, hypotension, hyperglycemia condition. The delivery device must be assed as the low flow systems do not provide the adequate ventilatory requirement. The high flow system is used only when approved but meets or exceeds their Peak Inspiratory Flow Rate. Mechanical ventilation is associated with function of lungs (Perman, Goyal Gaieski, 2012). In this case the patient may ensured of high flow system. The patient is assessed for auscultation of breath and type of breath that is cough or deep. Cough is common in pneumonia, a noscomial infection. It is necessary to identify the wheezes, crackles, adequate inspiration and expiration time. Wheeze indicates the narrowed airway and respiratory distress. Sound indicates pulmonary congestion and impaired gas exchange indicates abnormal breathing. Narrowed airway, pulmonary ventilation, perfusion, is common in septic shock (sellars et al., 2016). Further, injuries to the chest wall are observed and assessed for pain, deformities, and movement of chest wall. The rationale is to rule out any clot formation due to inflammatory process. Increased capillary permeability and Vasodilatation also hampers the bodys adequate perfusion indicating sepsis. The chest pain was found to be 5/10 and may be due to clot. Fever and increased heart rate may be due to systematic inflammatory response syndrome trigged by activated immune system (Zhang, Chen Ni, 2015). It is also evident from the high respiration rate evident from the primary assessment. According to Silversides et al. (2015), presence of sepsis leads to systematic inflammatory response syndrome. Heart rate, temperature and respiratory rate are important criteria to confirm infection in respiratory tract. Two of the above criteria is fulfilled and is confirmation for sepsis. It rules out any other major insult to the body such as myocardial infarction. Septic shock is further confirmed by the hypoxemia, hypotension, and oliguria, as they are signs of organ dysfunction (Van Vught et al., 2016). Pathophysiology of septic shock Sepsis is defined as a life threatening organ dysfunction which occurs as a result of infection. The infection may be caused from a variety of conditions such as post operative treatment procedures or due to incomplete cure from a variety of medical conditions. Under sepsis conditions the blood pressure falls below the abnormal level and sufficient oxygen are not transferred to the vital organs of the body leading to ac condition of multiple organ failure (Avni, Lador, Lev, Leibovici, Paul Grossman, 2015). The septic shock leads to tissue perforations where the capillaries start to leak resulting in organ failure. It is defined by persistent hypotension requiring vasopressin to maintain a mean arterial pressure of 65 mm hg or higher and a serum lactate level greater than 2 mmol/L. The signs and symptoms of sepsis vary within the patients such as fevers, chills, rigors, confusion, anxiety, difficult breathing, nausea, vomiting etc. As mentioned by Damiani et al. (2015), the sepsis is further triggered by a variety of clinical conditions such as the presence of chest and pulmonary infections. One of the major features of septic shock is the occurrence of peripheral vasodilatation where the normal mechanisms to vasoconstrict fail in the smooth muscles. Septic shock is further triggered by conditions such as pneumonia. It affects the blood coagulation system resulting in tissue leakage. As mentioned by Liu et al. (2014), sepsis has been mostly recently related to being immune-stimulated. Under sepsis schok cellular changes becomes activated in a deteriorating fashion leading to lymphocyte apoptosis. This disturbs the blood coagulation homeostasis resulting in blood to clot when it should not, resulting in clogging of vessels, which further restricts the blood flow in the patient. The dysfunctional vasculature results in local bleeding. Additionally, the tissue perforations results in the contents between the cells to mix, resulting in organ failure by disrupting the normal homeostasis of the body (Semler, Andrews Bernard, 2018). The sepsis shock results in the production of high amount of interleukins which result in the development of hyper inflammatory responses. The tissue in jury also enhances the levels of endotoxin which further triggers the release of cellular cytokines. The release of cellular cytokines further induces widespread secondary infection. The excessive activation of TNF ad endokines would result in suppression of activity of neutrophils, which fail to engulf the phagocytic cells (Andrews et al., 2017). The disturbed homoeostasis also affects the normal mechanism of apoptosis by delayed removal of the neutrophils and hastened removal of the lymphocytes, which further enhances the rate of the infection resulting tissue lyses. The respiratory syndrome has been associated with decreased chemotactic responses, which could be related to disrupted homeostasis of the cell. One of the most serious concerns of the septic shock is the occurrence of multi-organ failure which could enhance the chances of mortality in the patient. The pro-inflammatory and the anti-inflammatory responses lead to mitochondrial or endothelial dysfunction (Semler, Andrews Bernard, 2018). The series of septic shock also affects the normal neurotransmission in the patient. This results in loss of sense regarding time and surroundings and result in the development of utter confusion in the patient. This situation has also been referred to as encephalopathy with sepsis. In this respect, administration of glucorticoids has been seen to relive the condition of sepsis within the patient (Gawlytta et al., 2017). However, the results are debatable as no assured benefits have been obtained for the follow up of the method. Pathophysiology of experienced clinical manifestations The clinical condition of the patient could be studied over here in order to understand the deep rooted pathophysiology of the patient. The patient here depicted a number of clinical conditions as an associated symptom of septic shock. Some of these have been studied in details in order to understand the current health scenario pertaining the patient. Catherines past medical conditions shows that she had a history of cardiac ischemia. Therefore, the disturbed homeostasis owing to sepsis could have hampered the rate of neurotransmission resulting in delayed responses across the sinoatricualr (SA) node of the heart (Toan et al., 2018). The patient recorded a respiratory rate of 36 breaths/ mins. The septic shocks lead to tissue and blood vessels blockage resulting in disrupted flow of oxygen. The patient was admitted to the hospital with decreased air entry to right lower lobe. Additionally, the sepsis shock could further aggravate the condition of the patient leading to airway block. One of the most common clinical symptoms expressed in sepsis is the lock jaw syndrome which could have fatal consequences. As mentioned by Montull et al. (2016), most of the times the lock jaw happens owing to secondary infections caused by pathogens , which gain entry into the body of the patient through catherer port channels. The doctor further examined her symptoms based upon nursing scales and standards such as PEARL which refers to Pupil equal, accommodating and reactive to light. The pupil dilations were noted around 3 mm which signifies a state of anxiety or confusion in the patients. The IDC drainage bag of the patient has 30 ml of urine output which further confirms the presence of oligourea. Additionally, the medical emergency team has suggested the administration of 500 mg of azithromycin to the patient in a 2 L of normal saline solution. The purpose for the admini stration of Azithromycin was to regulate the spread of the sepsis shock infection in the patient. As mentioned by Singer et al. (2016), the use of azithromycin has been associated with more ICU free days for sepsis with or without pneumonia. The sepsis shock and resultant tissue perforations resulted in blood coagulations s directly cutting off the oxygen supply in the patient. Therefore, the patient had been feeing breathlessness. On increasing the oxygen supply the state of panting in the patient could be reduced. Therefore, the patient was put upon increased oxygen supply in order to reduce the chances of a septic shock. As commented by Andrews et al. (2017), the restricted blood flow cuts off the required amount of glucose to the brain resulting in neural shock. Therefore, administration of glucose in standard normal solutions may prevent the chances of a severe septic shock. Therefore, the glucose control acts as substantial protection of the endothelial cells. Therefore, at any point care should be taken to avoid the administration of steroids to the patient. It has seen to produce lethal consequences in the patient resulting in high mortality rates most of the times. Nursing interventions Based on the assessment and the evaluation the prioritisation of care involves oxygen supply followed by fluid resuscitation, blood culture tests and lastly antibiotic therapy. The patient may be provided with significant respiratory support. It includes monitoring and providing adequate oxygen supply. High flow system is used only when approved but meets or exceeds their Peak Inspiratory Flow Rate. In this case the patient may be ensured of high flow system. Mechanical ventilation is associated with function of lungs. The goal would be to maintain SpO2 greater than 94% (Gaudry et al., 2018). It will help reduce the shortness of breath and maximise the oxygen level for cellular uptake. When resting the patients head on bed will be elevated and the position to be change frequently. It will help lower the diaphragm. It will promote aeration of lungs by chest expansion. It will promote the mobilization and expectoration of secretions. It will ensured by nurse that the patient is conscio us and the airway is clear by regular monitoring. Active intolerance is common in this condition due to decreased gas exchange. Therefore, the care will be clustered to conserve the patients energy. It is needed for essential task like ambulation, eating, deep breathing and coughing (Silversides et al., 2015). The patient may then be provided width N/S infusion. A massive fluid resuscitation is required in patient with sepsis. As per instructions from the medical emergency team 2L of Normal Saline 0.9% STAT will be started. A large amount of fluid may be required to maintain the tissue perfusions as the oxygen intake is already low (Silversides et al., 2015). It will help support the circulating volume and overcome hypovolemia and reduce fever diaphoresis (Rastegar, 2015). Adequate precautions will be taken to prevent fluid over resuscitation. According to Semler, Andrews Bernard (2018), sepsis is a combination of arterial dilation, venodilation, intravascular volume depletion, therefore, care will be clustered to address each problem. Prior to commencing antibiotics the blood test may be conducted, as antibiotics may alter the blood test results. For Catherine, appropriate labs shall be obtained that includes antibiotic troughs, ABGs and sputum cultures. For Catherine two sets of blood culture will be taken from separate sites. It will used to check for coagulations, blood glucose level and FBE. The next assessment will comprise of lactate (venous blood gas). The rationale is to assess CRP, LFTs and ELIC. The cultures will be taken without delaying the antibiotic administration. Senior clinician will be informed immediately if unable to access IV or obtain blood cultures. Ensure the blood cultures will be properly examined for identification of appropriate pathogen. Identification of organism is crucial to effective septic shock treatment. The timings of the collection of the lab reports are essential as related to administration of antibiotics and ensure accuracy (Urden et al, 2017). These interventions will be immediately followed by the administration of the antibiotics as the responsibility of nurse. The patient will be assessed for penicillin allergy and use of antibiotics previously that may have caused any side effects. The same will discussed with concerned physician. In case of renal impairment the dosage may be reviewed. Antibiotic therapy will also be discussed with ID service. Currently as per the Medical emergency team advice, Azithromycin- 500 mg will be administered on the right time and in right dosage to ensure efficacy. The use of azithromycin has been associated with more ICU free days for the patient affected with severe sepsis with or without pneumonia. Broad spectrum antibiotics will be used such as methicillin, but the dosage will be balanced against the renal function clearance. If MRSA is detected then vancomycin may be preferred for its efficacy (Hagen et al., 2017). Followed by the above interventions the patient and the family members will be educated on energy conservations, symptoms requiring emergency care, food and nutrition and effective airway clearance as well as coughing and breathing. It is necessary create awareness among patient on these aspects of recovery. It will ensure adherence the treatment protocol and compliance to medication (Viera, 2016). To evaluate the effectiveness of the treatment, and determine the next steps, it is imperative to draw the labs promptly. It is very sensitive in sepsis treatment. Therefore, the lab studies for the neutrophils and band counts will be monitored. Bodys response to the infection is indicated by the elevation of band cells. The room temperature may be decreased as the patient has high fever. Catherine may be provided with the antipyretics and the cooling blankets to save her from chills and shivering. The nursing interventions may further involve continuous monitoring of temperature. The patient may be assisted with tepid sponge bath. According to Zhang, Chen Ni (2015) septic patient has fluctuating temperature. It is essential to maintain the body temperature as an increase in the temperature increases metabolic oxygen demands. The patients cardiac output may be monitored as indicated. The patient will be monitored of the urinary output as the decrease in urine output indicates hypovo lemeia associated with vasodilation (Viera, 2016). Further evaluation may comprise assessment of hemodynamic status, nutritional status and fluid intake and output. The patient may evaluate the patient for hemodynamic stability. It must be evaluated if the patient can verbalise the feelings and understanding of disease process. Based on the findings appropriate discharge and home care guidelines will be provided (Urden et al., 2017). The case study analysis of Catherine has developed insights into the pathophysiology of septic shock and the cause of clinical manifestations. It helped develop the care plan based on prioritisation. The first priority is supplying adequate oxygen followed fluid volume and then performing blood tests to determine the antibiotic therapy. New cultural ideals and scientific findings, such as evolution, clashed with the religious philosophies of the time. The early Victorian age also had an influence upon literature and poetry and produced many excellent writers, such as Robert Browning, Charles Dickens, and Matthew Arnold. The subject matter of most Victorian works by these authors was usually socially oriented and focused upon the practical problems of daily life and contained moral messages for their readers. Robert Browning focused his poetry on the scandals of everyday Victorian urban fife. He used sex, violence and moral hypocrisy as themes in many of his poems. Browning, like Charles Dickens, filled his literary works with people from all levels of society and he also included characters that were immoral and evil. According to The Literature Network, Robert Borrowing dramatic monologues covered a wide array of subjects, from lucid dreams to the nature of art and even the meaning of existence. His poems Porphyrys Lover and My Last Duchess, are similar in that they both include murderers who coldly describe their evil deeds without any remorse. Porphyrys Lover, begins with a lover describing the arrival of Porphyry, and then it quickly descends into a description of her murder at his hands. He describes how he strangled his lover with her own hair to preserve the moment forever. The poem My Last Duchess also echoes this theme of depravity. The Bishop Orders His Tomb at Saint Parades Church, is one of Borrowing most famous poems and demonstrates his view that base, immoral tendencies could be found anywhere in Victorian society. This poem also portrays a man of high religious standing, who lacks the morals and values of Christianity. In the bishops final hours, he is not interested in seeking God or what the afterlife has in store for him, but rather he focuses on the material goods he is leaving behind on earth. He does not look at the afterlife as a chance of being closer to God, or coming whole, but rather sees it as his chance to leave a legacy on earth. In his final moments he is concerned over the type of gems and stone that his tomb will be made of, and is mostly concerned about outdoing Gondolas tomb. Rather than honoring the Christian idea that in dying on earth, one will thereby become alive to God, the Bishop focuses on honoring himself and his life. The bishop clings onto life, so that he can ensure that his tomb will reflect his greatness, and be an earthly monument that shows how much better he is than Gondola. When Browning addresses morality, his themes tend to revolve around the separation of people and morality from the church. Both the Bishop in The Bishop Orders His Tomb and the monk in the Soliloquy of the Spanish Cloister are concerned with earthly matters as opposed to the Christian message that one needs to focus on the things in Heaven. Browning uses art as a symbol of the loss of morality. The art mentioned in Borrowing poems sound like beautiful works; however, the beauty is merely an illusion that highlights a narrator who is immoral, at best, or a cold-blooded murderer at worst. The portrait of the Duchess was only painted because of the corruption and evil of the Duke. Just as the Bishops beautiful tomb was only to be constructed in order for the Bishop to praise himself, and his worldly possessions. Matthew Arnolds poems, Stanzas from the Grandee Chartreuse and Dover Beach, show his distrust of the past and the philosophies, theologies and ideals that were popular in their time. Arnold is living in an era of change, and while he determines that faith and religion no longer provide answers or meaning to his life, he has nothing to replace it with. In Stanzas from the Grandee Chartreuse he finds that he is wandering between two worlds (Arnold 85); the world of religion which he has deemed as dead (Arnold 85) and the new scientific knowledge and beliefs that are unable to fill the void left by the loss of faith, and are too powerless to be born. (Arnold 86) Arnold laments that if the time for faith and spiritual fulfillment has passed, then why cant science take away, At least, the restlessness, (and) the pain (Arnold 103). He is cynical about his own time period and questions the ability of society to make spiritual or moral sense of the new modern era. Arnolds concerns for future generations reflect the turmoil caused by booming industrialization. He suggests that religion should provide moral guidance and a social safety net during chaotic times of change, and although scientific knowledge and advancements appear to make the concept of faith obsolete, there will always be the necessity for of the nineteenth or even the twentieth century captures the isolation of modern humanity as does Dover Beach, with its use of the Tachycardia night battle. Anderson points out that the poem encompasses Arnolds theme about the human indention and attempts to capture the sense of confusion and isolation. When Arnold refers to the Catholic Church in Stanzas from the Grandee Chartreuse he mentions how the church and priests are prideful, and the purpose of their hymns is to honor the march of Rome (Arnold 599 line 50). Arnold uses imagery, specifically the scenery of nature as symbols of lost morality. Arnolds description of the long disused trail to the Saint Laurent Monastery symbolizes the abandonment of religion and religious morality and values in society. Arnolds description of nature seems to be violent and dark, symbolizing the clash of nature and religion. Charles Dickenss novel, Hard Times, was published in 1854 and serves s the authors commentary on industrialization and the age of machinery, as well as the prevailing philosophy of rationalism, self-interest, and fact. F. R. Leaves classifies it as a moral fable (Dickens 364), and states that Dickens is Possessed by a comprehensive vision, one in which the inhumanities of Victorian civilization are seen as fostered and sanctioned by a hard philosophy, the aggressive formulation of an inhumane spirit (Dickens 365). Louisa brother, Tom, also a product of strict utilitarian schooling and upbringing is described as someone Whose imagination had been strangled in his cradle and is a hypocrite and a monster (Dickens 102). Tom turns out to be a self-absorbed thief who prefers money and gambling above all else, even family. His life ends badly after he robs Bounders bank and eventually dies thousands of miles away from Louisa. Dickens communicates the loss of holding innocence due to the changing philosophies and times, and the impact the loss of childhood had on ones moral character. Dickenss characterization of Mr.. Grandkids good friend, Josiah Bounder, exemplifies the social and moral decline created by industrialization and capitalism. Bounder appears to be a self-made man who has worked his way up the social of a factory and a bank which he professes to be the result of being a hard worker who possesses self-discipline and boundless determination. Dickens exposes Bounder as a fraud and describes him as a Bully of humility, who had built his Indy reputation upon lies, and in his boastfulness had put the honest truth as far away from him (Dickens 196). Stephen Blackball represents all those that are righteous and good. He is caught between the corruption of Bounder and he corruption of the union, and ultimately his death is caused by the immorality of Tom Grandkid. That organization then uses those goods, combined with their own, to produce a final product. I adapted aspects of this approach but felt that a transformational approach was more appropriate for my t eam and my practice. Whilst the transformational model of leadership may be seen as dynamic and therefore appeal to change agents, in this scenario the proposed change is largely transactional. The evening clinic will not provide staff with new skills or knowledge and it is unlikely to be seen as a particularly innovative as it is simply expanding an already existing service within the practice area. However, it is important to recognise the need for transactional change, in this instance practical concerns of an evening well baby clinic are being addressed. The evening clinic will not only benefit service users, it will improve service delivery and help achieve both local and national requirements. 2.2 I feel that in order to look at management and leadership theories, it is important to understand the differences between them. Warren (2005) differentiated between management and leadership by stating that the main difference is vision. Leadership is concerned with vision, communication and values whereas management is primarily concerned with analysis, planning and problem solving. Kotter (1990) also suggests that both leadership and management are needed within complex organizations in order for them to run smoothly. This is supported by Marquis and Huston (2006) who state that the roles of the manager and the leader can and should be integrated and that it is essential for both approaches to be present within nursing. I believe that it is vital for leaders to have the ability to be both managers and leaders at the same time in order for quality of care to take place. It is important to remember that management and leadership are very different but have overlapping functions ( Ellis and Hartley, 2005, Gopee and Galloway 2009). To have aspects of leadership and management skills are an essential part of the SCPHN role. Gopee and Galloway (2009) support Huber (2010) about the key importance of a visionary approach to leadership. It is important to reflect on the differences between leadership and management, to have the ability to utilise management in order to enhance my leadership and promote flexible, positive and appropriate team development. Critically analyse leadership styles and apply them to the complexity of the delivery of care. 3.1 In the last year as a SCPHN I have been able to observe many different leadership styles, on reflection I believe that I have chosen aspects of these styles to develop my own style. One theorist suggests that leadership in a clinical setting influences followers to bring about improvements in care (Welford, 2002). Through research I have found that there are many different leadership styles, Hersey et al (2008) identified these styles as authoritarian, laissez-faire and democratic. Within my career I have encountered many of the leadership styles, this has enabled me to choose aspects of these styles within my own practice. I found the laissez-faire approach of no interference and lack of decision making and a lack of structure to be confusing and unclear. The advantages of this approach with groups are that they are fully independent and promote professionals working together (Huber, 2010). The authoritarian approach from previous leaders has been very directive and not team foc used. I found this approach did not encourage togetherness and therefore I would not want to promote this within my team. In conflict situations I can see how it would be an efficient approach. I aim to be a democratic leader who works with there team, sharing responsibility and decision making although this may be a long drawn out process I believe it will facilitate an improved project. Huber(2010) stated that the challenges of a democratic style are getting people with different professional backgrounds to work together and decide on a plan of action. To overcome this I ensured that the team shared common goals. I created motivation within the team to examine working practices. This was confirmed to me as many of the staff showed their interest by their offering of ideas to meet this challenge. By tapping into the moral dimension of a proposed change i.e. promoting the need to contribute in order to protect the safety and health inequalities for those children and families who wo uld not otherwise be in a position to attend a well baby clinic during the day. It was also recognised that there may be resistance to working unsocial hours. As two health visitors will be required for each clinic and there are currently in excess of 20 health visitors employed by the trust they may only be expected to cover one clinic every 10 months. Some staff may even volunteer to work more often providing relief for those staff who arent keen to cover the clinic while providing a benefit to themselves if they can start work later in the day, therefore demonstrating that the democratic leadership style further inspired staff to change by motivating followers to transcend their own self-interest for the sake of the team and organization (Bass 1985). 3.2 Situational leadership was developed by Hersey and Blanchard (1977) and assumes the leader adapts their style according to a given situation. Encouraging team input and facilitating problem solving are key features of the supportive behaviours exhibited by the situational leader (Northouse 2004). This style has two main types of intervention: those which are supportive and those which are directive. The effective situational leader is one that adjusts the directive and supportive dimensions of their leadership according to the needs of their subordinates (Northouse 2004).As most team members were highly motivated in the project, freely offering suggestions and ideas, a directive role was not needed. The supportive behaviours I employed encouraged a participative approach characterised by the use of finely tuned interpersonal skills such as active listening, giving feedback and praising (Marquis and Huston 2000) which can be likened to a Skinnerian approach of positive reinforceme nt. 3.3 I can identify my correct use of the democratic leadership style by working with and alongside team members encouraging participation. This is achieved by assessing workers competence and commitment to completing the task. The member of staff that appeared to take little interest and was not able to offer ideas displayed a lower developmental level compared to other team members and hence I directed her more using the coaching behaviours advocated by Hersey and Blanchard (1977). This coaching promoted inclusion and participation by: giving encouragement, soliciting input and questioning the participant on what they thought of the proposal and the changes they would like to see. This was done to increase levels of commitment and motivation (Northouse 2004) and thus integrate that team member into the change process. On reflection this can also be identified as an example of reducing the resisting factors to the change within the force field as by adapting to the needs of that team member, she was encouraged to take part and share ideas rather than hinder progress and potentially thwart the change. I aim to develop my leadership style further by gaining feedback from my team members and by reflecting on what have been positive and negative experiences, whilst maintaining a link with best evidence based practice. 3.4 When implementing my evening well baby clinic and introducing my new leadership style, it was important to remember that change would be needed. When proposing change it must be recognised that if a structured process is not used the process could easily fail (Keyser and Wright,1998). It is important therefore, to acknowledge the complexity of the process. Lewin (1951) identified three stages in the process of change, these were unfreeze, move and refreeze. Within this scenario, theunfreeze process would include communication and planning with both staff and PCT management in order to gain their backing and support for the evening clinic. The move process would involve trying the evening clinic for a period of six months, and observing its effectiveness during this time. If the evaluation of the service proved it to be successful it would ultimately result in the clinic becoming established concluding the refreeze process. Through implementing the clinic I gained peoples thoughts and opinions and what they felt was needed, in Kassean Jagoos study (2005), they identified the unfreezing stage as that of facilitating peoples thoughts on the current situation. Sheldon and Parkers (1997) research found that people can only be empowered by a vision that they understand and that it is paramount that strategies are used to foster inclusion and participation so that all team members are fully aware of the impetus for change. 3.5 When improving care, two potential obstacles have been highlighted by Tait (2004), these were limited resources and the pace of change. With these in mind, a force field analysis (Lewin, 1951) has been completed to try and identify potential barriers. The issues I have identified are that it must be established early in the process the arrangements for the remuneration of staff time. Possible options are overtime payments or time owing. Staff will display individual preferences depending on their individual perception of the benefit of each option. As the decision on how time will be paid will lie with the PCT management team it is important to establish their response early as this issue is likely to be raised by the health visitors very early in the change process. Another issue identified was Health and Safety. As the building is already used for a family planning Clinic, any health and safety issues are already likely to have been addressed. I however considered it to be good practice to revisit and review the risk assessment. I identified a training issue around securing the health centre at the end of the evening, these locking up and safety procedures could be addressed with a short in house training session. After identifying the above issues it was my responsibility as a leader to consider resources and budgets available in order to achieve all my aims and objectives. 4. Identify and evaluate areas of leadership that enhance and benefit the quality of client care. 4.1 The Government has clearly outlined the need for nurses to develop leadership skills at all levels within the workforce in order to deliver the NHS modernisation programme (DH 1998; DH 1999). The leadership role expected of community practitioners is evident in Shifting the Balance of Power (DH 2001a) and Liberating the Talents (DH 2002) with the expectation that health visitors will lead teams which will deliver family-centred public health within the communities they work (DH 2001b). I strongly believe that by collaborating with other agencies when setting up my evening well baby clinic I have improved the quality of care for clients within my practice area. 4.1When implementing my project I took into consideration the felt and expressed needs (Bradshaw, 1972) of service users, and in line with both local Primary Care Trust (PCT) and government policy (Sec 2.3) regarding accessibility to services, it is proposed that an evening clinic be introduced for a trial period of six months. I made this decision as a leader of my team to ensure quality care and provision was implemented. To enable ongoing quality and evaluation change will be audited and evaluated in order to inform future practice and service delivery. In health visiting I believe that the emphasis should be placed on quality of care, providing and promoting access to health information and helping people make sense of the information so that they are able to make informed lifestyle decisions (DH, 2000). 4.2 A study investigating parents preferred sources of child health information found that when parents required advice on their childs general health care needs, the child health clinic was the second most popular source of information and advice (Keatinge, 2005). Child health nurses were identified as a good source of information, parents felt comfortable talking to the nurse and advice was seen to be reliable. Attendance at the child health clinic was viewed as an opportunity to obtain regular information and advice (Keatinge, 2005). A study of parental satisfaction with the health visiting service found that approximately two thirds of health visitor contacts took place in the clinic and routine weighing and general advice accounted for a high percentage of recent contacts in one year old infants. Again the health visitor was viewed as an important source of advice (Bowns, Crofts, Williams, Rigby, Hall and Haining, 2000). The National Service Framework (NSF) for Children, Young P eople and Maternity services (DH, 2004), contains several standards. Standards 1-3 are particularly relevant when considering a well baby Clinic, they focus on promoting health and identifying needs, supporting parents and having services centred around the family. Each of these standards can be addressed in a well baby clinic. The NSF is intended to lead a cultural shift which will result in services designed around the needs of the family, not the needs of the organisation, thus resulting in quality of care for all (DH, 2004). 4.3As a leader it is essential to have an awareness of clinical governance to ensure health care organisations can develop cultures and ways of thinking in order to improve quality of care (Tait, 2004). I have considered the culture of the organisation within which the proposed change will take place as I felt it was important. In my experience of the health visiting service, individual health visitors cover individual caseloads and generally work independently. This is not to say that a team culture does not exist but communication is essential, and as a leader I can facilitate this as part of my role. In addition to this regular health visitor meetings and annual away days encourage communication and help foster the wider team spirit. Clinical governance attempts to provide joined up policy development (Tait, 2004) so it is important to note that the issues highlighted are high on both local and national agendas. As a result of this it is hoped that the proposal, attempting to impr ove service provision with minimal resource implications is likely to be given serious consideration by service providers. Initial consultation with the management team was sought to identify if there is managerial support for the proposed evening clinic. Once this was achieved the process of consultation with health visitors and administrative staff began. It is hoped that by encouraging shared governance and shared leadership the proposal will be both practitioner owned and organisationally supported (Scott and Caress, 2005). 5.Demonstrate a dynamic and flexible approach to leadership issues. 5.1Within the project there is a mixture of cultures that have proved beneficial when planning the expansion of the well baby clinic. Managerial support was established early in the process, so that the change would be less opposed. However in addition to this staff were encouraged to contribute their ideas and concerns the change process may progress more smoothly. The implementation of this strategy reduced the risks of potential conflict. Barr and Dowding (2010) state that by being a dynamic and flexible leader who is able to resolve conflict effectively, high quality patient care can be achieved. Change can sometimes be viewed as a negative thing. A percentage of the team who will be affected by this change are established health visitors. There can at times be apathy to change and a tendency to continue with a certain practice because it has always been done that way or because something has been tried and failed before. 5.2 If conflict was to arise within my team I would use a conflict resolution strategy as identified by Barton (1991). This approach can be adapted by leaders to help improve team moral and productivity (Huber 2010). I believe the important factors for the leader to implement are effective communication, assertiveness and empathy. If this technique is delivered effectively I believe can be resolved quickly and with minimal upset. If conflict arises and a leader avoids confronting an issue or withdraws from the situation this can be beneficial as it allows for a cooling off period between team members but I believe that this is not a solution as it will not resolve the conflict. Marquis and Huston (2006) support Hubers research by saying that a leader should address conflict but also needs to recognise and accept an individuals differences and opinions. Therefore a flexible leadership style should be adopted whenever possible. 6 Conclusion and Summery. 6.1 In conclusion I feel that a model which places great importance on the needs, values and morals of others is transformational leadership (Northouse 2004; RCN 2005) and elements of this could be identified in my leadership. The goal of transformational leadership is to create a vision change what is into something better. Although transformational leadership did not originate within the nursing profession, its usefulness is in its application towards implementing the proposed change in practice. Transformational leaders are accustomed to sharing power, using influence and developing potential and are seen as the only leader likely to implement lasting change (Marriner-Tomey 2004). 6.2 Before completing the process I was inclined to believe that a large proportion of change was dictated to staff by managers and that as an individual member of a large work population I had a relatively little influence over work practices and few opportunities to lead other staff. The positive outcome of compiling this report has been gaining insight into the process of change and that different types of change and leadership are equally important. I have also benefited from actually completing the process and analysing the potential problems that may occur when trying to introduce a change in practice. I feel that the knowledge gained has influenced and inspired me to strive to become a motivational and beurocratic leader. 6.3 The negative points have been seeing how much work is required to bring about a relatively small change in practice. This process has taught me that in my career I will be unable to change everything I want to. It has also been difficult gathering the evidence base which has been frustrating as this appears to be a fundamental indicator in ensuring a proposal within practice is taken seriously. In the future I hope my new confidence in my ability to lead and empower will make me a valuable contributor to the health visiting service. I will carefully study those working practices I would like to change, ensure there is a good evidence base for any proposals and follow a structured process in order to maximise the potential success of future ventures. 6.4 Evidence based practice- leadership-SCPHN.
